How to manage a team remotely

I've been working from home very happily for two years and I love it. My work is either building The Copy Club community, collaborating with the small team and our fantastic volunteers, or working as an outsourced CMO, building very strong relationships with internal teams. It's all been about managing teams and building focus when we are not all around the same desk. 


Last week I gave a talk on everything I've learnt and there was really positive feedback (humble brag), but some great questions about how to manage teams, so I am going to collate my specific thoughts.
